The Mark VIII, Marauder and earlier SN95 Mustang GT (later ones were 8 bolt, not 6) flex plates all match this description. I even have one for sale.

Any Crown Vic 05 and newer has a dual pattern for the torque converter (for 12 inch and 11 1/4 inch) and is 164 teeth and 6 bolt crank. Part number is 4W7Z-6375-AA

Only the Windsor 4.6 SN95 GTs used the 8 bolt flexplate, they were all 6 bolt from 96-98 and 01-04, including the 03/04 Mach 1 auto
